Rationale, aims and objectives To compare the pharmacotherapeutic approaches to diabetic hypertension of family physicians (FPs) and general practitioners (GPs). Methods A retrospective prescription-based study was conducted in 15 out of a total of 20 health centres, involving 115 primary care physicians – 77 FPs and 38 GPs, representing 74% of the primary care physicians of Bahrain. Prescriptions were collected during May and June 2000 to comprise a study population of 1266 diabetic-hypertensive patients. Results As monotherapy, angiotensin-converting enzyme (ACE) inhibitors (...) (37.9%) and β-blockers (38.3%) were the most commonly prescribed classes of antihypertensives by FPs and GPs, respectively. Calcium channel blockers (CCBs) were ranked third by both categories of physicians. For two-drug combinations, a β-blocker and an ACE inhibitor was the combination of choice for both physician categories. Patients managed by the FPs were more likely to receive a β-blocker–CCB combination (17.4 vs. 14.9%) or a diuretic–ACE inhibitor combination (16.7 vs. 11.4%) and less likely to receive a β-blocker–diuretic combination (11.8 vs. 16.7%) than those managed by the GPs. The proportion of patients receiving antihypertensive combinations was 40.6 and 38.5% for FPs and GPs, respectively. While the GPs prescribed CCB as a monotherapy to the elderly most often, the FPs choice was a β-blocker. Diuretics were less preferred by both FPs and GPs. β-Blocker–ACE inhibitor was again the most preferred combination of both FPs and GPs. FPs prescribed CCB–β-blocker combinations more often than GPs (P = 0.01), whereas CCB–ACE inhibitor combinations were less preferred (P = 0.09). A trend towards excessive use of short-acting nifedipine as monotherapy for elderly patients, both by FPs and by GPs, was noticed. Glibenclamide, alone or in combination with metformin, was the foremost antidiabetic drug prescribed by FPs and GPs. Middle-aged (45–64 years) patients seen by GPs were more likely to receive glibenclamide than those treated by FPs (P = 0.001) and less likely to receive gliclazide (P = 0.01). Combinations of a β-blocker with either glibenclamide or insulin were prescribed considerably more often by GPs.Conclusions Within the same practice setting, a substantial difference was observed between FPs and GPs in terms of preference for different classes of drugs in the management of diabetic hypertension. The compliance of both FPs and GPs was suboptimal; overall, the compliance of the FPs was closer to the recommended guidelines, however. Because of continued controversy over the role of Aristotelianism in theology during the second half of the thirteenth century, many Scholastics of Aquinas's generation analyzed the quiddity of sacramental character from an Aristotelian perspective, working to describe the doctrine inherited from the Church's tradition using Aristotelian terminology. As will be shown, although some argued that sacramental character should be understood as a habitus, Aquinas argues that character is better understood as a power.The second and third sections of this article will explore the development of Scholastic accounts of the quiddity of sacramental character during the medieval and early modern periods, and the wider theological implications of these conversations. In the final analysis, it is my aim to show not only that Aquinas's account of sacramental character is intrinsically fitting on theological grounds, but that Aquinas's account of sacramental character as a qualitative power conveys and preserves the received theological tradition more effectively than the options proposed by some of his contemporaries. In Latin America Cardenal is generally regarded as an enduring poet. He brought a recognizably Latin American material into his poetry, and he introduced to Spanish-language poetry in general such poetic techniques as textual collage, free verse lines shaped in Poundian fashion, and, especially, a diction that is concrete and detailed, textured with proper names and the names of things in preference to the accepted poetic language, which was more abstract, general, and vaguely symbolic. But what is notable in Spanish-language (...) poetry is not only Cardenal’s “craft,” in the sense given this word by Seamus Heaney to mean manipulation of poetic resources; there is also this poet’s “technique,” which in Heaney’s sense means a “definition of his stance toward life.”2 Cardenal’s characteristic poetic stance has been admired because he addresses the political and social pressures that shape—and often distort, damage, or destroy—life and feeling. This is apparent even in the earliest poems Cardenal has chosen to preserve. “Raleigh,” for example, is a dramatic meditation from 19493 in which the treasure-hunting explorer marvels at the expanse and wealth of the American continents and out of sheer pleasure recounts some of the triumphs and hardships of his travels. Although his alertness and wonder make him sympathetic, this Raleigh’s vision of the New World as a limitless source of wealth is forerunner to the economic exploitation of the land and people.One might ask, What are the political and social circumstances which, rather than distorting and damaging life and feeling, nurture and preserve them? Perhaps one might answer that, paradoxically, destructive conditions of life have many times proven insufficiently powerful to prevent the creation of poetry. And some poetry has even arisen in reaction to the destructive: such conditions produce resistance, which, if it cannot heal the spirit, can lend it strength. One might answer further that it is not Cardenal’s or any artist’s responsibility to establish what circumstance will form a fruitful matrix for art, but only to work as honestly and as hard as political, social, and artistic circumstances will permit. 2. I claim that the revisions John Rawls made to his theory of justice—as seen in his political conception of justice as fairness in the revised edition of Political Liberalism and Justice as Fairness: A Restatement—result in him being able to secure justice for all persons even in their private lives. Thus, I defend his theory against common communitarian and feminist criticisms, viz the lack of moral community and inability to secure justice for individuals in the private domain. I demonstrate that (...) justice is secured from the categorical requirement that all associational life and moral doctrines must adhere to the constraints of justice, by (what Rawls terms) the indirect application of the first principle of justice. The implications of and necessity for this indirect application are precisely what I interrogate in this thesis. I suggest that this indirect application requires unconditional acknowledgement and internalisation of the values of liberty and equality (political values of the first principle) into all aspects of civil society. I claim that Rawls realised this necessity for the congruence between the moral powers of persons, the spheres of society, and the right and the good and, as such, developed a complementary relation between these three features. Coherence between these three aspects is essential; without coherence, citizens would have a split moral personality, the public and the non-public spheres would be in conflict, and the political values and the moral doctrines of individuals would undermine one another. The significance of this strict coherence is that it can ensure the adequate development of individuals’ capacity for citizenship during the three stages of moral development. This results in citizens being able to form bonds of civic friendship necessary for justice as reciprocity to be realised. Thus, in this thesis, I position my argument against theorists such as Nancy Rosenblum, who argue for a more relaxed congruence between political institutions and civil society, as according to her illiberal forms of associational life can help form the necessary bonds of love and care needed for mutual cooperation in a liberal society. In opposition to this, I claim that strict congruence is necessary; without it, a stable liberal society is not possible as the citizens would not be able to able to respect one another on free, equal, and mutually beneficial terms. I rely on Hochschild’s sociological evidence to demonstrate that civic friendship has not been developed, due to a lack of reciprocity, impacting the negative development of one’s sense of justice. I then illustrate how this lack of reciprocity is amplified by digital and artificial intelligence (AI) technologies. The significance of this thesis is that I claim, firstly, Rawls’s last revisions and works before his death demonstrate that justice is a collective non-comprehensive good that results by constraining individuals’ associational life and their partial comprehensive doctrines to Rawlsian demands of justice. Thus, Rawls takes seriously the feminist and communitarian critiques he received. Secondly, I claim that this interpretation of Rawls’s theory of justice cannot be conceptualised in terms of a binary presentation of the priority of the right versus the good debate and, instead, I argue for a reconceptualisation of the priority of the right versus the good as ranges on a continuum of structural relations between the right and the good. In recent years, the "science of science" has combined computational methods with novel data sources in order to understand the dynamics of research communities. As the name suggests, science of science is primarily focused on science and technology, with less attention to the humanities. However, many of the questions investigated by science of science are also relevant to academic philosophy: To what extent can the discipline be divided into subfields with different methods and topics? How are prestige and credit distributed (...) across the discipline? And how do these factors interact with other factors, such as gender, to shape job market outcomes? In this paper, we provide some empirically-informed answers to these questions by applying computational methods to job market data for Anglophone academic philosophy. We find, first, evidence that is consistent with the analytic-continental divide, but is also consistent with other, more complex ways of organizing academic philosophy into distinct intellectual traditions; second, a clear prestige hierarchy, dividing PhD programs into two distinct prestige categories; and third, evidence that gender, prestige, and country have notable effects on academic job market outcomes for recent philosophy PhDs. In 2008, Samantha Newbery, then a PhD student, discovered a hitherto confidential document: ‘Confidential: UK Eyes Only. Annex A: Intelligence gained from interrogations in Northern Ireland’ (DEFE 13/958, The National Archives (TNA)). It details the British Army’s notorious interrogations of IRA suspects that led to the eventual banning of the ‘five techniques’ that violated the UK’s international treaty obligation prohibiting the use of torture and ‘inhuman or degrading treatment or punishment’. Having decided that the document – Intelligence gained from should (...) be put in the public domain, she invited me, along with the eminent lawyer, Philippe Sands and Brian Stuart, a former Secretary to the British Joint Intelligence Committee, to contribute commentaries. I concluded that even if the document was accurate in both what it stated and in what it left out, that use of the ‘five techniques’ (tantamount to torture) was unjustified, since the normalisation and institutionalisation of torture is far worse than failing to obtain the information thus gained. Whether or not this joint paper will make a difference to UK policy concerning the use of torture remains an open question; whether or not it should is not. This piece is an example of how my book on torture, deliberately written for a public readership, has made it possible for me to make interventions in practical policy, or at least to attempt to, whether in occasional writing for eg The Chartist and the web-based Open Democracy, or directly for activist groups, eg my ‘Ethical Opinion’ for Physicians for Human Rights – Israel, Report: Holding Health to Ransom: GSS Interrogation and Extortion of Palestinian Patients at Erez Crossing (2008). ISBN: 978-1-4985-9765-4.It has been just over three decades since the last book-length engagement with aesthetics in Bonaventure's work (S. McAdams, "The Aesthetics of Light: A Critical Examination of (...) Bonaventure's Doctrine of Light in View of His Aesthetics," [PhD diss., Pontifica Universitas Gregoriana, 1991]). Heartily welcome, then, is Thomas McKenna's Bonaventure's Aesthetics. The purpose of the book, in McKenna's words, is "to provide a comprehensive analysis of Bonaventure's aesthetics, that is, his philosophy, theology, and mystical theology" (2). In so doing, he intends also to argue for a resolution to a list of disputed questions raised by contemporary voices – most famously that of Balthasar – in the debate on Bonaventure's aesthetics. Put another way, McKenna's project has two aims: first, to understand Bonaventure's aesthetics on its own (historical, theological, philosophical) terms; second, to determine whether that aesthetic connects significantly to contemporary questions in the field of aesthetics. In this review I will detail and engage critically with some of the foundational methodological moves McKenna makes in the book's introduction. I will then summarize the big themes of the book's four chapters, and conclude with a few evaluative remarks.However extensive their merits, earlier studies on Bonaventure's aesthetics are not as thorough as McKenna's and, more problematically, are limited by their investment in predominantly Enlightenment-era concerns. What I mean is that thinkers such as Shaftesbury, Baumgarten, and Kant were absorbed with matters such as the immediacy of the aesthetic experience, disinterest, the role of the affect, freedom of the imagination, and the categorization of les beaux arts. It is this trajectory (call it "the standard paradigm," using McKenna's terminology) that the field of aesthetics, as we know it, has largely followed. Early historians of aesthetics viewed, and often evaluated, medievals through the standard paradigm. Even later, more revisionist historical work sought to understand medieval thought in distinction from the paradigm. In most instances, [End Page 243] though, it is the Enlightenment-era list of concerns that serves as the fulcrum on which historical work has tended to swing. Our understanding of Bonaventure, then, is limited to the extent that we take the standard paradigm as guide.Bonaventure's Aesthetics is so compelling, in part, because it does not begin with the standard paradigm, but rather a more historically sound (and frankly, more interesting) list of research questions. This is not to say that McKenna rejects the standard paradigm. It is simply too deeply embedded in how we conceive of such matters to naively ignore. Instead, McKenna works to situate these later concerns as secondary, or parallel, to Bonaventure's concerns and interests. McKenna realizes that he faces a difficulty at this juncture, one that must be addressed by anyone aiming for historical sensitivity regarding their object of study. The difficulty is what McKenna calls "the hermeneutical crisis," and he formulates the key issues by quoting Jean Grondin: "The basic doctrine…is that every particular phenomenon must be conceptualized within the context of its age" to avoid anachronism. But if so, this "raises a striking epistemological problem…Our view of earlier ages must itself be produced by reference to our present, and is thereby relativized." Thus Grondin asks, "how if at all, is it possible to escape from the hermeneutic circle of our historicity?" (9, quoting J. Grondin, Introduction to Philosophicaal Hermeneutics [New Haven: Yale University Press, 1997], 76).Restricted to Bonaventure studies, McKenna poses the worry this way: "is it possible to accurately recover the conceptualization of beauty and, perhaps, the arts of any age – ancient, medieval, or modern – with a reasonable degree of accuracy? Either God's giving efficacious grace to an individual (...) is such as to cause that individual to act a certain way, merely in virtue of giving that grace to that individual, or it is not. If that grace is not intrinsically efficacious, then you need to explain the way in which God can cause an action by giving grace which does not require grace to be intrinsically efficacious. On the latter front, Molinists aim to affirm that human beings are free even while acting under the influence of God's grace, and hold that God's grace is not such as to make it that one could not have done otherwise. They explain this by appeal to counterfactual truths about what human beings would do, metaphysically prior to and independent of God's subsequent decisions to give or withhold grace. These "counterfactuals of freedom" are central to Molinist accounts of freedom, since the truth of these counterfactuals alone secures the fact that human beings are acting freely even when God gives them grace and causes them to perform certain actions. Those human beings could have acted otherwise. Conversely, since God knows these truths, he can use them in order to give grace to an individual at those times when he knows it will be efficacious.To me, the Molinist view is a non-starter. What these truths consist in, what grounds them, how God uses them—I find the critiques compelling [End Page 675] and Molinist answers to these questions metaphysically problematic.1 I am committed to finding a Báñezian solution, therefore, to the riddle. From my perspective, the mystery of predestination invites confusion ultimately because the issue is a subtle philosophical, not a theological, problem. The resolution to the riddle of grace and free will can come only through an adequate account of the nature of human freedom. And I remain unsatisfied by a lack of philosophical clarity among Báñezian authors on the nature of freedom. In a recent paper, I therefore posed a problem for Báñezianism that resembles what is called the "grounding problem" for Molinism: where do the truths about alternative possibilities come from? And I illustrated the problem in the context of the account of grace given by one famous defender of the view, Fr. Réginald Garrigou-Lagrange, whose work in turn was recently promoted by Taylor Patrick O'Neill.In short, Garrigou-Lagrange appears to accept that there need to be alternative possibilities if human action is to be free. He gives an accounting of those possibilities as follows. On one side, when I sin, Garrigou-Lagrange argues that God's denial of efficacious grace, even though this denial occurs prior to anything I do or could have done, does not necessitate my sin because I performed the sin by means of my normally operating volitional and intellectual capacities (which God merely refrained from interfering in), and I had alternative possibilities provided by the possibility of God providing efficacious grace. Then, on the other, when I do good, he appeals at many points to non-culpable "defects" regarding my intellectual or volitional capacities as grounding those alternative possibilities that I could have sinned even when acting under efficacious grace. In both cases, my acts are still metaphysically contingent. However, neither case provides a sufficient grounding for alternative possibilities (I argued), because I had no relevant control over God's decision to deny efficacious grace, nor were those "defects" in light of which I could have sinned under my control. Instead, I argued that Garrigou-Lagrange's account of humans having alternative possibilities for action would be successful only if there were facts about what I would have done under sufficient grace, truths prior to and independent of God's decision to give efficacious... Existing research has examined if undergraduate factors influence chemistry and physics, or physical science, doctoral degree entry and whether variables during PhD programs associate with graduation. Yet research on the transition from bachelor’s degree to doctoral degree entry (i.e., PhD entry in less than 6 months, attainment of a master’s degree prior to doctoral degree entry, or working in a science-related job for more than a year prior to doctoral degree entry) on PhD degree graduation remains scarce. Our study examines (...) the transition from bachelor’s to doctoral degrees to see if experiences therein associate with female PhD graduation, after doctoral degree enrollment. Our logistic regression analysis, of female chemistry and physics doctorates (n = 867), indicated that attainment of a master’s degree did not change the likelihood of graduation, when compared to direct entry into physical science doctoral programs. Meanwhile working in a science-related job for a year or more is associated with a significantly lower likelihood of physical science doctoral graduation when compared to women who entered directly into PhD programs or received a master’s degree prior to enrollment. Coaching is a systematic and goal-oriented one-on-one intervention by a coach aimed to guide clients in their professional and personal development. Previous research on coaching has demonstrated effects on a number of positive outcomes, including well-being and performance, yet little is known about the processes that underlie these outcomes, such as the type of questions coaches use. Here, we focus on three different types of coaching questions, and aim to uncover their immediate and sustained effects for affect, self-efficacy, and goal-directed (...) outcomes, using a between-subjects experiment. One hundred and eighty-three medical residents and PhD students from various medical centers and healthcare organizations in the Netherlands were recruited to participate in a self-coaching writing exercise, where they followed written instructions rather than interacting with a real coach. All participants were randomly allocated to one of three conditions: either one of two solution-focused coaching conditions or a problem-focused coaching condition. Self-report questionnaires were used to measure key outcomes of coaching, that is positive and negative affect, self-efficacy, goal orientation, action planning and goal attainment. Two follow-up measurements assessed if the effects of the self-coaching exercise led to problem-solving actions within an initial follow-up period of 14 days and a subsequent follow-up period of 10 days. Findings showed that participants experienced more positive affect, less negative affect, and higher approach goal orientation after the solution-focused coaching exercise compared to the problem-focused coaching exercise. In all conditions, goal attainment increased as a consequence of the self-coaching intervention. We discuss the implications of our findings for the science and practice of contemporary coaching. Friends, family, and the Association of the Advancement of Philosophy and Psychiatry community mourn the death of Osborne "Ozzie" Wiggins this past May 18. In many ways, his story contributes a large portion to the founding of the AAPP, this journal, and the philosophy/psychiatry community worldwide.I met Professor Wiggins as a sophomore at Southern Methodist University in Dallas in 1974. I was a student in his twentieth-century humanities class.
